Referrals

Some insurance carriers require a referral to obtain services rendered by Annapolis Allergy & Asthma. It is the patient’s responsibility to obtain these referrals and present them at time of service along with any applicable co-pay. Obtaining a referral for any additional services may also be required. If the referral is not present at time of visit, the appointment may be rescheduled. Co-payments will be collected at time of visit.

Annapolis Allergy & Asthma accepts most major insurance carriers. Please refer to the listing below. If your insurance is not listed, you may call your carrier to verify our participation. We do not participate with Maryland Medical Assistance, but offer services for self pay patients. Payments will be due at time of service.

Aetna, Alliance, BCBS, Beechstreet, Carefirst, Cigna, Coventry, First Health, Fidelity, Great West, Healthnet/Tricare, Informed, Johns Hopkins Health Plans, Kaiser, Mamsi, MDIPA, Medicare, NCPPO, Optimum Choice, PHCS, Preferred Health Network, Railroad Medicare, Tricare, United Healthcare.
